I created Formation Physical Therapy + Performance, because you should never be just a number. You are a person with goals and dreams. I am here to help you heal, move strong, and get back to doing what you love — without fear of getting hurt again.
You deserve a physical therapist who listens and cares. I will make a plan that fits your body, not just a one-size-fits-all plan. I started Formation because I believe you should get the right care, not just what insurance limits us to.
Before I was a physical therapist, I worked as a school psychologist. That means I know healing is not just about your body — it’s about feeling seen, heard, and strong inside too.
I work with active adults who want more than a quick fix. If you are ready to train smart, move better, and feel strong, I am ready to help.
I have played a lot of sports — softball, dance, track, flag football, ultimate frisbee, CrossFit — and even tried pole dancing because of some awesome clients! If I have not tried your sport yet, I will learn it. I want to know how your body moves so I can help you even more.
When I’m not working, you might see me lifting weights with my wife, at the beach, or cheering for the Hokies and the Seattle Storm!
