FORMATION is located in the heart of Carytown in RVA. Our address is 8 S Crenshaw Ave, Richmond, VA 23221.
Please park on the street, or in the lot across the street. Do not park in the McDonald's parking lot.
If I’m working with someone at the time you arrive feel free to have a seat near the front door and I’ll be with you as soon as I’m finished.

Our bread and butter is helping athletic adults with back pain get back to feeling good and strong with exercise and life.

Before your first visit you will receive an Evaluation Acceleration Form which will include information regarding current and previous injuries and health history. This allows us to get to work quicker on your first visit.

Please don’t. It’s hard to assess movement and expose joints in jeans. Please wear clothing you can work out in. At FORMATION we prefer:
Shorts, yoga pants, or stretchy pants.
Light, stretchy t-shirt, tank top, sports bra.
Shoes to work out in.

No! Virginia is a “direct access” state which means you do not need a prescription for physical therapy. This allows you to receive treatment faster and cut out extra steps, such as waiting for an appointment with your physician. Reach out to start getting help today.

While most of our clients have health insurance, FORMATION is an out-of-network provider. You are in control of your care, free from the boundaries imposed by insurance. This allows us to provide one-on-one, hour long visits with you and you alone. We have learned that insurance-driven physical therapy results in having less time with your physical therapist, decreased service options, and denials of treatment. We choose to be better. We will work together to create a plan of care that meets your needs without third party limitations.
At this time we are unable to provide physical therapy to clients with Medicare, and Medicaid does not reimburse for services.

Most clients are reimbursed for part of their visit by their insurance if they have met their deductible. Ask your insurance company what your out-of-network benefits are for physical therapy (we can’t guarantee reimbursement). FORMATION will provide you with a bill with all the information you would need to submit.

Yes! We accept FSA, HSA, credit card, cash, or check.

Most clients feel significant relief within 2–4 visits. Sometimes even faster.
But relief is just the beginning. If you want the pain to stay gone, we need to rebuild what caused it in the first place.
That’s why, after those first few visits, we typically see clients weekly to every other week. Between sessions, you’ll have:
A fully customized home program (delivered via app)
Unlimited messaging + check-ins between visits
Clear movement cues, video demos, and real-time feedback
So even if you’re not in the clinic every few days, you’re still progressing every single week.
This structure gives you flexibility and results, while building real independence.

Pelvic floor physical therapy focuses on improving how the muscles of the core and pelvic floor work together so you can move, exercise, and live without symptoms like leaking, pain, or pressure.
At Formation, pelvic health care is integrated into a full-body approach. Your visit may include:
Movement assessment and strength testing
Core and breathing coordination work
Strength training and progressive loading
Education about pelvic health and training modifications
Hands-on treatment or soft tissue work if helpful
Our goal is not just symptom relief; it’s helping you return to training and daily life with confidence.

An internal exam may be offered, but it is never required.
Internal assessment can sometimes provide helpful information about pelvic floor muscle strength, coordination, and tension. However, many pelvic health concerns can also be addressed through:
external assessment
movement evaluation
strength and coordination training
If an internal exam is recommended, your therapist will explain exactly what it involves and answer any questions beforehand.
You are always in control and can choose what you are comfortable with.

Pelvic floor physical therapy can help with concerns such as:
leaking with coughing, running, or lifting
pelvic pressure or heaviness
core weakness or instability
pelvic or hip pain
pain with intercourse
pregnancy and postpartum recovery
Many of these symptoms are common, but not something you simply have to live with.

Pelvic floor symptoms affect women at many stages of life — including athletes, lifters, runners, and women who have never been pregnant.
We regularly help women address pelvic health concerns whether they are pregnancy-related or not.

In most cases, yes. And we encourage it.
Our approach focuses on helping you train with your body, not against it. We’ll guide you through modifications and strength progressions so you can continue building strength while addressing the underlying issue.
